Interested in becoming a New Patient?

Before we can schedule a new patient appointment, we will need to gather some information from you including:

*Medical Insurance: Please have the name of your insurance, as well as the ID number.
   *Once you book your Appointment, please Contact your insurance to update your PCP.
*Previous Medical Office: We will need this information to request records.

*Your current Medication List: This information will be entered into your chart for review.

*The date of your last office visit: To ensure we are compliant with insurance.

Office visits are usually by appointment only. At times, we may allow walk-in sick visits, but there may be a longer wait. We try to maintain as precise a schedule as possible for all of our appointments. Prior to your visit, it is your responsibility to confirm that we are a participating provider with your insurance plan. Please arrive a few minutes ahead of your scheduled appointment time. We may have to reschedule a non-emergency appointment if you arrive late. If you are unable to keep an appointment, please call to cancel it at least 24 hours in advance. There is a fee of $35 for missed appointments not cancelled with 24-hour notice.


Our telephones are answered Monday through Friday during normal operating hours. 
To better facilitate your care, we utilize a voicemail system. Our trained staff will triage your calls. Providers are on call for emergencies after hours and weekends.
If you have a life threatening emergency, dial 911. For all other emergencies call our office at (978) 256-5522 for instructions.

Be ready for Your Visit:

  • Verify your insurance company has us listed as your PCP.

  • Bring your insurance card(s) and prescription benefit card(s).

  • Bring your co-pay listed on your insurance card.

  • Bring your medicines or list of medicines.

  • Check your medicines to see if you need refills.

  • Bring your glucose meter if you have one.

  • Make a list of questions you may have for your provider.

  • Bring records of recent care received by other providers.
